Clinical Interest

Anti-nuclear antibodies (ANA) are autoantibodies directed against components of the cell nucleus. By extension, they include antibodies recognising certain components of the cytoplasm and cytoskeleton. Along with rheumatoid factors, ANAs are sought whenever a non-organ-specific autoimmune disease is suspected.

ANAs can be divided into antibodies directed against extractable nuclear antibodies (ENA), non-extractable nuclear antigens and antigens located in the cytoplasm.

A positive ANA or ENA often leads to more specific tests, such as anti-dsDNA, anti-Sm, anti-RNP, anti-Ro/SSA and anti-La/SSB, anti-Scl-70 or anti-J0-1 antibodies to differentiate the various autoimmune diseases.

  • Systemic lupus erythematosus (SLE), with over 95% of patients testing positive.
  • The specific antibodies associated are anti-double-stranded DNA (anti-ds-DNA), anti-SM, anti-RNP (Ribonucleoproteins), Anti-Ro/SSA and Anti-La/SSB.
  • Sjögren's syndrome, essentially with Anti-Ro/SSA and Anti-La/SSB.
  • Systemic scleroderma is often associated with anti-centromeres (ACA) and anti-Topoisomérases I (Anti Scl-70).
  • Mixed connective tissue disease (MCTD) is often associated with anti-RNP/Sm.
  • Dermatomyositis and polymyositis are often associated with anti-Jo-1
